ABOUT US
What is Community Support Services?
Five County Community Support Services (Community Action) works to reduce poverty, revitalize low-income communities, and empower low-income families and individuals to become fully self-sufficient. Programs are funded through the Community Services Block Grant (CSBG) and partner funding, and are offered across Beaver, Garfield, Iron, Kane and Washington Counties.
How to apply
- Download and complete the Community Action application (PDF) or the Spanish application. There is currently no online application.
- Call (435) 674-5757 or email css.online@fivecounty.utah.gov once your application is complete.
- Please allow 3 business days for a response. Walk-ins may not be served.
All programs are contingent upon available funding. Qualifying for or applying for services does not guarantee funding.
