Five County contracts with the Division of Aging & Adult Services for the administration of three in-home service programs which are funded by State and Federal dollars.  We are responsible for the administration, eligibility and case management of these programs and sub-contract with local providers for the direct services.   We are also a Medicaid provider with the Utah Department of Health to provide the case management services for New Choices Waiver Program which provides home and community based care for seniors and disabled adults transition from a nursing facility.  Details regarding these programs are listed below.  

In-Home & Community Based Programs

The waiver is designed to provide services to help  frail, elderly individuals receive the services they need to remain in their own homes as an alternative to care in a nursing facility.

The program is supported through a joint federal and state funded program called Medicaid 1915(c) Home and Community Based Services Waivers;  with administrative oversight through the Utah Department of Health & Division of  Aging & Adult Services.

Text Box: Aging Waiver Medicaid Program

The Alternatives program provides a comprehensive array of client centered services delivered through a variety of  home and community services that offer service delivery options to eligible recipients to enable them to continue to live at home. 

This program is supported through appropriation from the Utah State Legislature.  The Division of Aging & Adult services oversee the administration of the program at the State level

Text Box: State Alternatives Program

The Caregiver Support Program is funded through Title III-C of the Older Americans Act to help support in-formal caregivers.  Services include information, assistance, support, caregiver training and counseling for the caregiver.  In addition, respite & supplemental services are available for eligible caregivers to provide them with some relief  and Support from their ongoing caregiving duties. 

Division of Aging & Adult Services oversees the administration of the program as the State level.

Text Box: Caregiver Support Program

The New Choices Waiver Program helps eligible individuals to move out of  nursing facility settings.  Services are designed to help people transition out of a nursing facility and re-establish themselves back in the community. 

Five County Area Agency on Aging role with the program is to provide the case management services.    Administration of the program and all referrals go through the Long Term Care Bureau at the Utah Department of Health.
